Dear Veteran,

Your authorization for Virtual Lifetime Electronic Record (VLER) Health Exchange is due to expire this month. You previously provided your authorization for VLER Health Exchange which is a program that securely shares important parts of your Veteran health record between the U.S. Department of Veterans Affairs (VA) and participating non-VA health care providers. Sharing your health information enables your health care providers to access important information about your health history so they can provide you the best possible care.

In order to continue sharing your VA health care information over our secure and confidential network known as the eHealth Exchange, you will need to renew your authorization.  This program continues to be free and voluntary for Veterans, but every five (5) years it requires your authorization to participate. If you are a Premium* eBenefits account holder, we invite you to Connect Your Docs through the VLER Health Exchange. Here's how to renew your authorization through eBenefits to securely share your health records:
   1) To get started, go to https://www.ebenefits.va.gov/ and log in.
   2) Select Manage Health. 
   3) Scroll to the Health Records row and select VA Health Record Sharing.
   4) Select Manage My Authorizations and Preferences and then select Start Authorization.
   5) Accept the terms and conditions (you must click Yes to continue), then select Save and Continue.
   6) Mark the box and select Sign. You will then be prompted to re-enter your username and password.
   7) Finally, click Reauthenticate. (This next step may take a couple of minutes to process.)

You can also submit your authorization by completing VA Form 10-0485, which is available online at http://www.va.gov/vaforms/medical/pdf/vha-10-0485-fill.pdf. VA Form 10-0485 may be completed and printed, and then faxed, mailed, or hand carried to your nearest VA Medical Center Release of Information (ROI) Office.

When you "Connect Your Docs" through VLER Health Exchange, you reduce your need to carry paper records between appointments, give your providers immediate access to more health care information (such as during a trip to an Emergency Room), and reduce your chances of having duplicate tests done.
