The Patient Survey Reports functionality of the HT Reports module allows users to search on Veteran patient survey response data from the Health Data Repository (HDR) to produce the following patient survey reports:
To access the questions for the Patient Satisfaction Survey (V1.0 and V2.0)and the Veterans Rand SF-12 (VR-12) Survey, click on the links at the bottom of each report window (see image below). To download a MS Word document of the surveys, click on the survey link and then click on the Export button at the bottom of each question window.
