Distribution of Surveys allows users to generate a report on the total number of Home Telehealth survey responses for versions 1.0 and 2.0 of the Patient Satisfaction Survey. Users also have the option of selecting various report criteria to narrow the report by VISN, facility and modality (i.e., Home Device or IVR).
NOTE: Click on the links below the report to view the questions for V1.0 and V2.0 of the Patient Satisfaction Survey.
Click on HT Reports > Patient Survey Reports > Distribution of Surveys.
Specify the date range by entering the date in the From Date and To Date fields (using the "MM/DD/YYYY" format) or selecting the dates from the calendars.
Select either national, VISN, or facility survey statistics:
For national survey statistics, verify that the National Survey Statistics radio button is selected (this is the default for the report).
For statistics by VISN, select the VISN Survey Statistics radio button and the VISN from the drop-down menu.
For statistics by facility, select the Facility Survey Statistics radio button and the facility from the drop-down menu.
Select the Modality ("All", "IVR", "Home Device", or "Browser"). Note that the default is "Do Not Include".
The default for the survey type is "All Surveys". To narrow the report to one type of survey, select the radio button for the desired survey type (Patient Satisfaction V1.0 or Patient Satisfaction V2.0)
Click Generate Report.
The report window displays with a table containing the number of surveys by Vendor and the report criteria selected.
Click Reports Menu to return to the previous window to generate a new report.
