Veteran Authorization and Preferences (VAP) Phase 2 provides the veterans consent to exchange personal health information. VAP will provide support and maintain the continuity of care for patients; enhance the quality of care through the ability to access necessary health information rapidly at the point of care; Improve patient safety and reduce medical errors; and reduce or eliminate redundancy of procedures (e.g. laboratory or radiology tests), or otherwise improving the efficiency of care or of administrative processes.
