  1. You can add a new user  by entering the SSN  and Patient Last Name in the Select Patient from VA patient Panel

This  look up searches the Registries Master VA patient list, but it doesn’t search already existing Polytrauma records. If the patient you are trying to enter already has a Polytrauma record, you will get notified on the next screen.

If you cannot find the patient above you can enter the record manually by clicking on the Continue without selecting Patient button. You will be directed to the Add New screen. Here you can follow the steps below:

 

  1. You can fill in the information  by clicking the drop down menus, or by checking  your selection in the check boxes in the following panels: the Patient Information, Tracking, Trauma Information, Injury Details, Treatment, Follow Up Clinical Needs and or Follow Up Provider Information
  1. You can click the Save Button to save the information you updated. A pop up message will display to confirm ‘save successful’
  2. You can click the Close Button if you don’t want to save any updates to go to the main Polytrauma View Edit screen.
