This process lets you upgrade your My HealtheVet account to Premium status. You can complete and sign a paper VA Release of Information Form 10-5345a-MHV and send it by mail. You will need to be logged into either the AccessVA www.access.DNS /accessva or eBenefits www.ebenefits.DNS using your DS Logon Premium Username and Password.
After you have connected your accounts, you will be asked if you would like to start to upgrade to a Premium account. To begin:
1. On the Start Upgrading Your My HealtheVet Account Today page:
- Select the radio button: Yes, I would like to start upgrading my account Manually
- Select Continue
After you select Continue, you will be taken to the Manual Authentication Instructions page.
2. Print the Manual Authentication Instructions page. Do this by selecting the Printer-Friendly link at the top of the page.
3. Use this link to print, read, complete, and sign the VA Release of Information Form (10-5345a-MHV).
4. Mail your signed paper form to the Release of Information Office at your local VA health care facility. You can use the VA Facility Locator to find the address.
Note: The upgrade does not start until the VA receives your signed paper form in the mail. Please allow up to 20 business days to complete the upgrading of your account.
After your account has been upgraded to Premium, your status on your My HealtheVet Member Logon box will change to Premium
