This mobile application will expand the support to case managers/outreach coordinators in the field and their assessment and tracking of homeless Veterans by adding the capability to document assessments, interventions, and geo-spatial information during the conduct of field operations. This phase is intended to provide a framework for data entry in the field through a streamlined intuitive user interface that will synchronize with the HOMES database.
