The Clinical Case Registries (CCR) Enhancements application supports the maintenance of local and national registries for clinical and resource tracking of care for patients with several clinical conditions. At this time, the software supports 19 local registries. The available registries include Alzheimer's disease, amputation, breast cancer, cerebrovascular disease, chronic obstructive pulmonary disease, chronic renal disease, congestive heart failure, diabetes, dyslipidemia, hepatitis C virus (HCV), human immunodeficiency virus (HIV), hypertension, ischemic heart disease, low vision/blindness, mental health, multiple sclerosis, osteoarthritis, obstructive sleep apnea and rheumatoid arthritis. This application provides extensive clinical and administrative reporting capabilities for local medical center use with each of the local registries. CCR accesses several other VistA files that contain information regarding clinical visits, laboratory tests, hospital admissions, other diagnoses, patient demographics, prescriptions, procedures and radiology exams. This access allows identified clinical staff to take advantage of the wealth of data supported through VistA in on-going efforts to improve patient care. The Clinical Case Registries (CCR) Enhancements application also provides for automatic transmission of data to the VA's National CCR. At this time, two national registries concerning HCV and HIV are available. These two national registries are used for extensive reporting on the state of VA care for these two populations, national improvement programs to continually improve care in these high cost conditions, data used by the Allocation Resource Center (ARC) for cost modeling as well as mandated reporting to the Departments of Health and Human Services (HHS) and the Centers for Disease Control and Prevention (CDC).
