This Requirements Specifications Document (RSD) and System Design Document (SDD) were developed for the Tracking of Abnormal Results (TAR) project for the Department of Veterans Affairs (VA) Transformation Twenty-One Total Technology acquisition program, known as T4. Women Veterans Health Strategic Health Care Group (of VHA Patient Care Services) supports a request to provide a Computerized Patient Record System (CPRS) tracking system for normal, abnormal and incomplete test results related to women’s health cancer screening services (i.e. mammograms). Additionally, a related request also seeks to implement a cancer clinical case registry in order to monitor and track Women’s Health cancer and diagnostic test results, facilitate coordination of care, and improve management and timeliness of treatment for all Veterans.

Currently, there is no effective way to monitor or track results (to include normal, abnormal and incomplete test results and breast cancer screenings such as mammograms) in CPRS, which can potentially cause unnecessary delay in, and lack of, appropriate follow-up care for patients. In addition, there is no system in place within CPRS that has the capacity to identify, monitor and track all women with normal, abnormal and incomplete mammogram results and assess sequential milestones in the diagnostic workup and treatment. The registry will have the ability to provide tools to assess a cohort of women and assess milestones in their care management. Paired with National clinical reminders and the proposed tracking of normal, abnormal and incomplete test results capability, this solution will implement a patient and population management tool which will improve detection, diagnosis, treatment and management of breast cancer in the Veterans Health Administration (VHA).
