You can initiate a search for a patient in either the Provider Portal or in the Patient Portal.

Enter one of the following criteria in the Patient Search dialog box:
Patient’s last name/partial name with a minimum of one character (NOTE: Do not include a space between the patient’s last name/partial name. Example: Doe,John)
Patient’s FMP and SSN
Combination of the first letter of the patient’s last name plus the last 4 digits of the patient’s SSN (NOTE: Do not include a space between the first letter of the patient’s last name plus the last 4 digits of the patient’s SSN. Example: D1234)
Once you have entered the search criteria, click Search or press Enter to begin the patient search. Patient Search will be performed against the source system (VA or DoD) that authenticated you upon log in.
A Patient Search can also be initiated from all subsequent pages of the iEHR web application.
